Form Designer

Form Designer allows users to extend their auditing database to capture additional audit-related information. The administrator will be able to create customized fields such as :

1. Radio Buttons 

2. Drop Down Lists 

3. Check Boxes 

4. Tables ( Static and Dynamic) 

5. File Uploads

6. Text Fields 

The Form Designer will manage the back-end expansion of your database.

E.g., Assume you need to capture specific CDI information that is very specific to a contract or facility; your ReviewMate Administrator can extend the database by creating forms on account or per audit level.
